Abigael Home Health Agency provides In-home care without the restriction of an
assisted living facility or a nursing home. The agency affords clients the opportunity to age
in place and minimizes the need for a transition to a higher level of care. We provide:

Care Management Services
Care Management Services provides clients with a qualified and experienced Registered Nurse who manages client care. They help with medication management, communication and coordination with doctors regarding appointments and prescribed treatments. They also help make recommendations for additional support services and conduct scheduled home visits to discuss changes or concerns with a client's care.
Nurse’s Aide/Assistants
Help with all the same services as Home Assistants but they’ll also help with personal care:
• Incontinent care,
• Transferring the client from bed, bath or car and more
Home Health Assistants
Help with all the same services as Companions, but also assist with:

We can discuss your care needs over the phone and/or we will arrange a time to meet with you and your loved one. We can refer caregivers to provide services ranging from short shifts to around-the-clock care, and we will strive to refer and match you with a caregiver who meets all the criteria that you specify. We work hard to ensure caregiver-client compatibility and, in most cases, care services can begin the same day.

CAN I CHANGE MY SCHEDULE IF MY NEEDS CHANGE?Yes, changes are easily accommodated. Client needs always come first. You might need more care after an acute event such as surgery, but as time passes your schedule might change so that less care is needed. Similarly, as care needs increase over time, we can assist in making the necessary changes. The client is in control of the caregiver schedule and we make adjustments as soon as reasonably possible.

WHAT ARE THE RATES CHARGED?Rates are based on the type of services you require, the hours needed, and the rate requested by the caregiver who is providing the services to you. All rates are based on fair market value. We strive to refer the highest quality caregivers and nurses for a very competitive price.
